Buy NAD+ injection material and you have not yet said what you are buying. The phrase covers a compounded intravenous bag, a shot given in a clinic, and a gram of powder sold under research-use terms, and those three differ in who is accountable, which documents exist, and which assay decides whether the material was fit for the purpose in the first place.

NAD+ is a coenzyme rather than a peptide, so the analytical questions are not the ones this site usually asks. There is no sequence to confirm and no counterion arithmetic to do. What replaces them is a single test that FDA has effectively named for us.

Sorting the formats first prevents most of the confusion in this market, because the criteria that apply to one of them are meaningless against another.

The inversion is the point. The bar rises as you move up that table and the verification available to the person paying falls. A clinic session is the most tightly regulated of the three and also the one where the buyer typically asks the fewest questions, because the setting does the reassuring. A powder order is the least regulated and the one where a document check is actually possible.

Why a purity figure does not answer the question

Three properties get conflated whenever material is described as clean. They are separate measurements with separate methods, and they fail independently of one another.

  • Chemical purity is what a chromatographic assay reports: the area of the main peak against total peak area, which describes how much of the detected material is the target compound rather than related substances. It is an area percentage, not a mass fraction of the container, and it is blind to anything that does not chromatograph under the method used.
  • Sterility is the absence of viable microorganisms. It is established by a sterility test on a preparation, not by an assay on a bulk powder, and most research material is not sold sterile and does not claim to be.
  • Pyrogen burden is the quantity of fever-inducing bacterial material present. Bacterial endotoxins are lipopolysaccharide fragments from the outer membrane of Gram-negative bacteria, and they are what a bacterial endotoxins test measures, reported in endotoxin units per milligram.

The third is the one that governs here, for a reason that is easy to state and easy to miss. Endotoxins survive the two things people assume deal with contamination. They are heat stable, so autoclaving a solution does not remove them, and they pass through the sterilising-grade filters that retain bacteria, so filtration produces a sterile solution that is still pyrogenic. Killing the organism does not remove what it left behind.

They are also active at quantities far below anything a purity assay would notice. A certificate can honestly report a high purity figure on material carrying an endotoxin load that matters, because the two measurements are not looking at the same thing. FDA maintains a questions and answers guidance on pyrogen and endotoxins testing precisely because this is a distinct discipline within release testing rather than a footnote to purity.

Which produces the single most useful sentence a buyer of this compound can carry: a supplier who publishes chromatography and identity confirmation but no LAL result has documented the easy half.

What FDA documented, and why it was about this compound

The endotoxin criterion is not a theoretical preference imported from pharmaceutical manufacturing. In a notice whose content is current as of October 2024, FDA reminded compounders to use ingredients suitable for sterile compounding and named the problem directly. The agency stated that it is aware of compounders using food-grade nicotinamide adenine dinucleotide sold by repackagers to make intravenous products, and that ingredients identified as food grade are not suitable for compounding sterile drugs without appropriate processing, due to the high risk of contamination with microbes and endotoxins.

It then described the consequence. FDA reported receiving adverse event reports following use of NAD+ injectable drugs, including severe chills, shaking, vomiting and fatigue, with some requiring medical treatment, and stated that these reactions are consistent with excessive levels of endotoxins.

Three things follow that are worth separating. First, the failure was upstream of the clinic: the starting material was food grade, and the people harmed were receiving an administered product. Second, the notice urges manufacturers including repackagers to identify clearly on the label any ingredient intended for use in foods or dietary supplements, which tells you that the labelling gap is real and that a buyer cannot assume a container states its intended use. Third, FDA had raised a comparable concern previously about the dietary ingredient glutathione used to compound sterile injectables, so this is a pattern in how supplement-channel material reaches syringes rather than a one-off.

The certificate lines that separate the two grades

A food specification and a pharmaceutical specification are both real standards. They differ in what they were written to protect against, and the difference is legible on a document.

That final row is the whole argument of this page in one line. It is also why the request to make of a supplier is specific rather than general: not send me the COA, but send me the certificate for the lot you would ship, and confirm whether it carries a bacterial endotoxins result. What research supply is not, stated plainly

Material sold under research-use terms is bulk substance with a certificate. It is not a sterile drug product, it has not been released against a sterile-product specification, and no regulator has assessed any batch of it. A supplier who publishes an endotoxin result has told you something real about the starting material and has still not turned it into a finished preparation.

There are consequently no preparation steps on this page, no quantities and no technique. Not as a compliance gesture: the sequence from bulk powder to something suitable for administration is exactly the sequence that FDA describes compounders getting wrong with this compound, and a web page is not where it gets described.

Grading the evidence for infusing it at all

Two claims about NAD+ are routinely presented as one, and they sit at very different levels of support.

The first is that NAD+ is central to metabolism. That is settled. It is the coenzyme carrying electrons through central energy pathways and the substrate consumed by sirtuins, PARPs and related enzymes, and its cellular roles have been reviewed in detail by Covarrubias and colleagues. That literature is overwhelmingly cell and animal work, and on this site it grades as mechanism rather than clinical evidence.

The second is that infusing the intact coenzyme produces a clinical benefit. That is not settled, and the human record is thin. The most directly relevant published human work is a pilot study by Grant and colleagues, which tracked plasma and urine NAD+ metabolites during and after a six-hour intravenous infusion in a small cohort. It is a pharmacokinetic pilot: small, uncontrolled, and designed to describe where infused material goes rather than to demonstrate that anything improves. Reading it is worth more than reading a clinic page, and it does not establish benefit for any condition.

Clinics market NAD+ infusions for a range of conditions. This site reports that they do and adopts none of it. The distance between a well-characterised coenzyme and a demonstrated clinical outcome is the largest open question on this page, and no supplier, certificate or format closes it.

What are people actually buying when they buy NAD+ injection?
One of three different things. A clinic sells an administered infusion, where the preparation has usually come from a compounding pharmacy and a clinician is accountable for it. A telehealth or pharmacy route can supply a compounded prescription preparation. A research supplier sells a labelled quantity of powder under research-use terms with a certificate of analysis and no sterile-product release. Those differ in who bears responsibility, what documents exist, and what the buyer is able to check before paying.
Why is endotoxin the decisive test rather than purity?
Because purity and pyrogen burden are different quantities and only one of them was implicated in documented harm. Bacterial endotoxins are lipopolysaccharide fragments from the cell walls of Gram-negative bacteria. They are biologically active at quantities far too small to appear as an impurity peak in a chromatographic purity assay, they are heat stable, and they pass through the sterilising filters that remove viable organisms. A material can therefore be high purity, sterile, and still pyrogenic. Only a bacterial endotoxins test, usually the LAL assay reported in endotoxin units per milligram, addresses it.
Can I buy NAD+ IV bags from a research supplier?
No, and a supplier offering one should be treated as a serious warning rather than a convenience. A compounded intravenous preparation is a prescription drug product made by a licensed pharmacy or a registered outsourcing facility for an identified patient. Research suppliers sell bulk material under research-use terms, which is a different legal object with a different document set. A storefront selling ready-to-infuse bags is claiming a regulatory position it almost certainly does not hold.
Does a NAD+ shot buy from a clinic avoid the endotoxin problem?
Not automatically, which is the uncomfortable finding in the FDA notice. The agency stated that it is aware of compounders using food-grade nicotinamide adenine dinucleotide sold by repackagers to make intravenous products, and that it had received adverse event reports of severe chills, shaking, vomiting and fatigue consistent with excessive levels of endotoxins. The question to ask a provider is which compounder supplied the preparation and whether the starting material was released against a pharmaceutical specification.
What should a certificate for injectable-intent material contain?
A lot number matching the container, a named laboratory, a report date, an identity confirmation against a reference standard, a chromatographic purity result with the method stated, a water content figure, and a bacterial endotoxins result in endotoxin units per milligram. A certificate that stops after purity and identity has answered the easy questions. A certificate with no lot number describes a product line rather than the material you were sent.
